We offer in-person and appointments at our Sarasota and Tampa offices,
and telehealth appointments throughout the states of Florida and Colorado.

We are located in building 3. Turn into the health complex off of Shade Ave. Building 3 is the second building on the left. The address on the front/top of the building is 2415.
